Welcome to the Sy’s Fund Application.
To qualify for funding through Sy’s Fund you must be:
- A resident of the United States
- Undergoing treatment or receiving medical care for cancer or chronic cancer related medical issues
- Between the ages of 18 and 39
Application and all documentation can be mailed to:

Once we receive your application, you will hear back from us within two weeks. If you have any questions in the interim, or have issues downloading the application, please call or email us. We aim to make this process as simple as we possibly can while covering all pertinent information. Thank you.
